Center Medical Director, Oak Street Health
At the Del Mar Primary Care Clinic in Aurora, Colorado, I lead clinical operations for a center serving a predominantly older-adult, complex-chronic-disease Medicare population. I manage an interdisciplinary team of primary care providers, health coaches, and care coordinators, and partner with corporate population health and product teams on care management platform development and analytics.
